(Alexandria, MN)  —  A 3M employee is dead after an incident at the company’s Alexandria facility early yesterday morning.  The woman got caught in machinery while working the overnight shift.  The incident is being looked into by the Minnesota Occupational Safety and Health Administration.  Officials with 3M said the company’s emergency response protocols were put into action and the police, fire department and OSHA were all notified.  The factory has been open since 1967 and OSHA records indicate it was fined for an equipment violation in 2013.
